New Jersey Form MW-562, field by field

This page goes through Form MW-562 (2/25) box by box. It draws on the Wage Hub contractor guide from the New Jersey Department of Labor (NJDOL), last updated March 2026, and the Prevailing Wage Act and regulations.

The short answer

Form MW-562 is New Jersey's two page payroll certification for public works contractors and subcontractors, issued by its labor department (NJDOL). Page 1 is the weekly payroll grid. Page 2 holds the certification, a fringe benefit table, and the signature block. The current footer reads MW-562 (2/25), so confirm the version on NJDOL's Wage Hub page before you use it.

  • NJDOL's Wage Hub news page says in an April 14, 2026 entry that a new MW-562 dated 2/25 adds check boxes for a no work week and that the older 6/23 version is still compatible with the system.
  • The form has ten numbered columns on page 1, and page 2 holds the certification, the 4(c) fringe benefit table, and an electronic signature box.
  • Under N.J.S.A. 34:11-56.29(b)(2), certified payroll records go to the public body and to NJDOL's website each payroll period, within 10 days of the date wages were paid.
  • NJDOL's Wage Hub contractor guide says to fill out the MW-562 PDF on a computer, save a copy, click Add Certified Payroll, and drag the file in to upload it.
  • NJDOL's public works FAQ says an original signature is required on the payroll filed with the public body, while the form itself includes a box to sign electronically.
  • For the fringe column, NJDOL's public works FAQ says to divide the annual cost of the benefits by 2,000 hours for each employee.

What the MW-562 is

The MW-562 is NJDOL's Payroll Certification for Public Works Projects, for weekly and final certification (forms page). Page 1 is the payroll grid. Page 2 holds the certification and a fringe table.

Under N.J.S.A. 34:11-56.29(b)(2), you submit certified payroll each payroll period, within 10 days of the date wages were paid, to the public body and to NJDOL's website. For jobs with federal rules, see New Jersey versus federal certified payroll.

Which version to use

NJDOL's updates page says, in an April 14, 2026 entry, that a new MW-562 dated 2/25 adds no work week check boxes and that the older 6/23 version still works. The same page links two different files. Its download link, under "latest version," goes to MW-562 (12_16B), the file read here, whose footer reads 2/25. Its upload tips tell PDF users to use the "most current version" and link a 6-23 file, which did not load when read on November 4, 2026. A 6-23 file that the public works FAQ links also returned Page Not Found that day. Confirm the version before you use it: the Wage Hub site links the 12_16B file, and NJDOL's Wage Hub help address is njwagehubinfo@dol.nj.gov.

The header boxes

BoxWhat to enter
Name of Contractor or SubcontractorYour company name, with the box checked for your role.
F.E.I.N., Business AddressYour federal employer identification number, 9 digits per the guide and the updates page, and your business address.
Project NameThe guide does not describe this box on the PDF. Its Add Project form, which creates the Wage Hub project record, says the project name should be the public body's official name for the project and to ask the public agency if unsure. A subcontractor does not enter a project, because the Wage Hub FAQ says the general contractor or public body does, so take the value from that project record.
Project LocationThe guide does not describe this box on the PDF. The project record's location field is the street address where the work takes place, and the guide's Add Project form says it cannot be a PO box.
Contract I.D. or Project I.D.The guide does not describe this box on the PDF. The project record has a Contract/Project ID, and the guide's Add Project form says the format varies between public bodies, so ask the public body if unsure.
Contractor Registration #The box label matches the guide's Contractor Registration number sign-up item. The guide lists the 10 digit reference ID as a separate sign-up item, and no NJDOL source read for this page says which one this PDF box takes. See contractor registration.
Payroll No.No NJDOL source read for this page defines this box. Site questions go to NJDOL's Wage Hub help address, njwagehubinfo@dol.nj.gov, per the guide.
Date Wages Due & PaidNo NJDOL source read for this page says how to fill this box. The statute counts the 10 days from the date wages were paid (section 56.29(b)(2)); the guide's submission history table shows a Date Wages Due & Paid column filled from the data you submit. That describes a display column in the Wage Hub, not this box on the form.
Week Ending DateThe date the payroll week ends, which the guide says the Wage Hub requires.

In the Wage Hub's manual entry form, the guide says FEIN, Project Name, Contract/Project ID, Business Address, and Project Location may be prefilled; email the help address about errors.

No Work Week

To report a week with no work, the guide (March 2026) says to check the No Work Week box, on the MW-562 form for PDF uploads and in the certification step of manual entry. The undated Wage Hub FAQ adds that in manual entry you complete all fillable fields, then click the box. NJDOL's updates archive says, in a January 8, 2025 entry, that selecting No Work Week makes the Employee details section inactive.

Final Certification

The guide says that when a prime contractor ticks this box, the project becomes Complete automatically, and for PDF uploads it says to click the Final Certification box on the MW-562 form to complete a final certification for the project. The Wage Hub FAQ separately covers taking a finished project off the main dashboard: it says you cannot remove it, and must go into the project and change the status from active to complete. NJDOL's updates archive says, in a January 8, 2025 entry, that a pop-up lets contractors change their status when they click the final certification check box. None of these says what a subcontractor's tick does. To ask NJDOL, email njwagehubinfo@dol.nj.gov.

The payroll grid, column by column

Page 1 has ten numbered columns, with two lines per employee, S and O.

ColumnWhat goes in it
1. Employee Name and AddressLegal first and last name, street address, city, and state, per the guide. Include owner-operators and corporate officers doing covered work (public works FAQ).
2. Job Title, Work ClassificationExamples: apprentice, journeyman, foreman; carpenter, mason, plumber.
3. Sex, Race, EthnicitySex: M, F, or N (non-binary). Race: W, B, A, N, I, or M, per the key at bottom left. Ethnicity: H or N. N means Non-Binary, American Indian or Native Alaskan, or Non-Hispanic, by column.
S or OS is straight time, O is overtime. In manual entry, the guide counts hours up to 40 a week as straight and hours over 40 as overtime. It does not say this for PDF uploads or address overtime under your wage determination.
4. Day and Date, 5. Total HoursDates as mm/dd under SU through SA, hours worked each day, then the line's total.
6. Hourly Rate of PayThe dollar rate, not less than the wage determination in the contract, which the public works FAQ says the public body must provide at award.
7. Gross Amt. Earned: This Project, This WeekNo NJDOL source read for this page explains the two boxes.
8. DeductionsFICA, Federal Tax, State Tax, Other (Specify), and Total. Other is any other paycheck deduction, per the guide.
9. Net Wages Paid for WeekNet pay paid to the employee, which the regulation's payroll record definition lists.
10. Total Fringe Benefit Cost/HourAnnual cost of the benefits divided by 2,000 hours, per employee. The public works FAQ says items such as use of company vehicles or cell phones, lodging reimbursement, or company-provided tools may not be credited toward the prevailing wage fringe rate. It also bars crediting statutory deductions, workers' compensation insurance, and any part of a benefit deducted from pay.
Page 1 footer: Check if additional sheets attachedNo NJDOL source read for this page explains this box.

What the form leaves out

The form has no Social Security number box. The regulation defines a payroll record as a form satisfactory to the Commissioner showing employee information such as name, address, and social security number (N.J.A.C. 12:60-2.1). That definition is general and does not name the MW-562.

Page 2: the certification and the fringe table

  1. Items (1) to (3). Workers were paid full weekly wages, with no rebates and only permitted deductions. Payrolls are correct, rates meet the wage determination, and classifications match the work. Apprentices are registered with the US Department of Labor and enrolled in a certified program.
  2. Item (4), fringe benefits. Check 4(a) for benefits paid to approved plans, funds, or programs and 4(b) for cash. The guide says both can be checked on the Wage Hub web form. Its fringe step (section 3) appears to correspond to item 4, but its numbering does not exactly match the paper form.
  3. Item (5) restates the 10 day rule (N.J.A.C. 12:60-5.1(c)1.i, in Subchapter 5 of the regulations on that page).

Complete table 4(c) if 4(a) is checked. Its header says amounts contributed per hour: divide 2,000 hours into the yearly cost per employee. The guide's fringe table describes a US dollar amount paid per category, with no per hour wording. The two wordings differ.

Signing the form

The last block says checking the box and typing your name is an electronic signature with the same legal effect as a written one.

The public works FAQ, undated, says the payroll filed with the public body needs an original signature, and NJDOL's July 17, 2024 press release says hard copy goes to the contracting body. Neither addresses electronic signatures. The statute says the public body and the department are entitled to unredacted certified payroll submissions, and that online submission to the department does not relieve the contractor of submitting to the public body (N.J.S.A. 34:11-56.29(b)(3)). Until the public body confirms in writing that it accepts an electronic signature, give it a hard copy with an original signature.

Getting the form to NJDOL and the public body

To upload the PDF, fill it out on a computer, save an unlocked copy (updates page), then use Add Certified Payroll in the Wage Hub, as the guide describes. For setup, see NJ Wage Hub registration.

The statute and the Wage Hub FAQ say the public body gets its own submission. The form's notice says that copy can go through the Wage Hub or other methods, so ask which the public body accepts. Late? See late or missed payrolls.

Questions

Which version of the MW-562 is current?
MW-562 (2/25), per its footer. NJDOL says 6/23 still works.
How do I report a week with no work on the MW-562?
Check the No Work Week box in the header, per the contractor guide.
Do I sign the MW-562 by hand or electronically?
The form and the contractor guide say an electronic signature has the same legal effect as a written one. NJDOL's undated public works FAQ says the payroll filed with the public body needs an original signature. No NJDOL page read here says whether the first satisfies the second, so get the public body's answer in writing or give it a hard copy with an original signature.
Can I change an MW-562 after I submit it in the Wage Hub?
Not the content. The Wage Hub FAQ says a submitted payroll cannot be changed, so submit a corrected one with the same week ending date; the last one for that date is treated as correct. NJDOL's April 14, 2026 update says a payroll sent to the wrong project can be moved if you email njwagehubinfo@dol.nj.gov.
